Form 1095 – Hardcopy Available Upon Request Only
In accordance with a federal law enacted on December 23, 2024, the Paperwork Burden Reduction Act, we are no longer required to automatically furnish the Form 1095-C to employees, retirees, and other individuals enrolled in the District’s health coverage, even if you opted to receive a hard copy during Open Enrollment.

What does this mean to you?
Your 2024 Form 1095-C will be posted online in Benefit Central by the IRS deadline. To view your form, go to top menu, select “Benefits,” and click on “1095 tax documents.” You may request to receive a hard copy of your 2024 Form 1095-C by contacting Human Resources at the following email or physical address:

If you request a hard copy of your Form 1095-C, we will mail it by the later of January 31 or 30 days after the date we receive your request. If you have any questions, you can contact the HR Benefits team at 425-783-8557.
